ABSTRACT

BACKGROUND:Excessive lateral pressure syndrome is often associated with lateral retinacular tension and radiographic patel ar tilt. CT scan displayed that lateral retinacular release can effectively correct patel ar tilt. OBJECTIVE:To study the effect of arthroscopic lateral retinacular release combined with intraosseous dril ing and decomposition in the treatment of excessive lateral pressure syndrome. METHODS:Thirty-two patients with excessive lateral pressure syndrome were treated by arthroscopic lateral release combined with intraosseous dril ing and decomposition. The Lysholm scoring system was used to evaluate the treatment effect. RESULTS AND CONCLUSION:The mean duration of fol ow-up was 12 months. After 1 month, pain of al patients was released or disappeared;after 1 year, pain of 26 cases disappeared basical y. Lysholm scoring system assessment showed 20 cases were rated as excellent, six cases were as good, four cases were as fair and two cases were as poor. The excellent and good rate was 83.6%. The patients’ subjective satisfaction rate was 92.8%. The results indicate that arthroscopic lateral release combined with intraosseous dril ing and decomposition is a good method to treat excessive lateral pressure syndromewere. It has the advantages of less trauma and rapid recovery. Patel ar decomposition has a good effect in the treatment of patel ofemoral pain associated with patel ar tilt outward and lightens articular cartilage degeneration without damage to patel ar cartilage surface.

ABSTRACT

Objective To investigate relationship between polyethylene wear and decrease of femoral offset after total hip arthroplasty (THA).Methods Ninety patients who had undergone unilateral THA were recruited in this study.Coreldraw 12.0 software was used to measure change of femoral offset on the basis of immediate anteroposterior radiographies of bilateral hip after THA.In the light of the decreasing value of femoral offset (F),all patients were equally into 5 groups:F<2 mm,2≤F≤4 mm,4<F≤6 mm,6<F≤8 mm and 8<F≤ 10 mm.At final follow up,some parameters about polyethylene wear were measured by using Coreldraw 12.0 software on the basis of anteroposterior radiographies of bilateral hip.Results The linear wear volume was 0.72±0.13 mm in group F<2 mm,0.78±0.11 mm in group 2≤F≤4 mm,0.87±0.09mm in group 4<F≤6 mm,0.99±0.09 mm in group 6<F≤8 mm,and 1.17±0.15 mm in group 8<F≤10 mm.The linear wear rate was 0.132±0.025 mm/yr in group F<2 mm,0.139±0.019 mm/yr in group 2≤F≤4 mm,0.159±0.029 mm/yr in group 4<F≤6 mm,0.183±0.032 mm/yr in group 6<F≤8 mm,and 0.213±0.023 mm/yrin group 8<F≤ 10 mm.The linear wear angle was 5.0°±3.5° in group F<2 mm,3.2°±3.8. in group 2≤F≤4mm,-4.3°±4.9° in group 4<F≤6 mm,-8.7°±4.1° in group 6<F≤8 mm,and -13.0°±4.7° in group 8<F≤ 10mm.For three parameters above,there were significant differences between groups,except between group F<2 mm and group 2≤F≤4 mm.Conclusion The femoral offset should be restored to its anatomic status in THA to avoide superior lateral displacement of center of the femoral head,which could result in uneven stress distribution and increase polyethylene wear.

ABSTRACT

Objective To investigate the preliminary clinical outcome following PCL-retaining total knee arthroplasty (TKA) with 0°or 5°posterior tibial slope. Methods From Jan 2008 to Mar 2009, 84 patients (84 knees) with osteoarthritis (OA) underwent primary PCL-retaining TKA operations by the same surgical team. Among them, 27 patients (7 males, 20 females, with the average age of 69.5 years) underwent TKA using a cutting block and intramedullary cutting guide designed to impart a 5° posterior tibial slope (group A); 57 patients (15 males, 42 females, with the average age of 67.4 years) underwent TKA using a cutting block and intramedullary cutting guide designed to impart a 5°posterior tibial slope (group B). The preoperative demographic data and functional data had no statistical differences. The postoperative tibial posterior angle, maximum extension, maximum flexion and Knee Society Score (KSS) were compared between the two groups. Results All patients were followed up 12~24 months with an average of 15.7 months. No paralysis, wound infection, prosthesis dislocation and loosening were found. The postoperative tibial posterior angle was 5.7°±2.1° in group A and 0.9°±0.6°in group B (P< 0.05). The postoperative maximum extension was 0.8°±0.3°in group A and 1.2°±0.4°in group B (P >0.05). The postoperative maximum flexion was 115.7°±4.8°in group A and 101.1°±5.6°in group B (P< 0.05). The postoperative KSS was 87.6±5.9 in group A and 83.3±7.2 in group B (P >0.05). Conclusion Increasing tibial posterior slope improved the postoperative maximum flexion, but not the maximum extension and KSS, in PCL-retaining TKA.

ABSTRACT

Objective To measure D-Dimer(DD) ,thrombin-antithrombin complex(TAT), lasmin-antiplasrain complex(PAP) and protein C(PC) in ARDS,to find the clinical significance of them,discuss the objective base in the early diagnosis of ARDS in the lab, and offer the serologic bases in the treatment and prognosis of ARDS.Methods 105 patients of ARDS were selected as the study group, and 105 people were selected as the control group, all of whom were healthy with no thrombus diseases. The venous blood of everyone in both groups was sampied,in order to take a quantitative determination of plasma prothrombin time(PT), prothrombin time(TT), kadin partial thromboplastin time(APTT), the amount of blood platelet, DD, TAT, PAP, PC. Results Serum concentratation of D-D, TAT, PAP was significantly higher in patients in ARDS group than that in control group(P<0.01).Serum concentratation of PC was significantly lower in patients in ARDS group than that in control group (P<0.01). Conclusion Measuring the concentration of DD, TAT, PAP and PC was very important, which not only did good to the early diagnosis of ARDS,but also had a clinic value.

ABSTRACT

Objective One of the characteristics of rheumatoid arthritis was multiple joints involved symmetrically. The purpose was to study the method of early diagnosis and treatment for the atypical rheumatoid arthritis involving solitary joint. Methods From April 1997 to December 2002, 5 cases which were 1 male and 4 female patients with an average age of 36.8 years (range, 23 to 47 years), suffered from the swelling and pain of knee joint, and were diagnosed as early rheumatoid arthritis. The interval of the onset to surgical treatment ranged from 2 months to 3 years with a mean of 16.2 months. The patients were evaluated with serum test, joint fluids examination and radiological imaging. They were treated by the synovectomy under the arthroscopy, while the appearances of the joint under arthroscopy were recorded, and the synovial tissue was sent to the pathological examination. The improvements of the joints symptoms and the range of motion were assessed at final follow up. Results During the operation, the light green and little muddy joint fluids were found in all patients. ESR of the patients ranged from 29 to 51 mm/1h with an average of 32.2 mm/1h, all of which were higher than normal value, whereas, 3 cases had positive rheumatoid factor and 4 cases showed significant higher ?2-MG. The radiographic findings were osteoporosis in 3 cases, soft tissue swelling in 4 cases and narrowed joint space in 1 case. The synovium were proliferated and enlarged in dark violet, there were creeping pannus and necrosis fibrin deposition in the cartilage. All of the patients were diagnosed as rheumatoid arthritis confirmed by pathological changes of the synovium. The patients were followed up from 4 months to 5 years (mean, 18.7 months). Pain released in 5 cases, swelling reduced in 3 cases, the range of motion improved by 19 degrees. Unfortunately, all of the patients developed later into typical rheumatoid arthritis after the mean interval of 10.9 months (range, 3 to 25 months), the symptoms were consistent with the criteria of American Rheumatoid Association . Conclusion Solitary joint with rheumatoid arthritis is rare and may be the early status of rheumatoid arthritis. The specific appearance under the arthroscopy prompt the early diagnosis of the knee rheumatoid arthritis, and synovectomy by arthroscopy may be benificial to release the symptoms of rheumatoid arthritis.

ABSTRACT

Objective To evaluate the efficacy of total hip replacement in patients with ankylosing spondylitis(AS) and to solve the problem often encountered during surgery. Methods One hundred and twelve hips of 98 patients with AS undergone total hip replacement were reviewed. The mean duration of follow up was 4.2 years(range, 1.5 to 8 years). The pre and postoperatively comparative study on pain, range of motion, correction of deformity and total function evaluation were conducted. Results Pain relief obtained in all but 4 hips. Mean range of motion was improved from 37.5? to 69.5? and the flexion deformity of the involved hip was corrected from mean 38.2? to 5.7?, all of the patients could take care of daily living by themselves. Complications included penetrating fracture of proximal femur in 4, sciatic nerve injury in 3, early dislocation of the hip in 2, disassociation of the acetabular component in 2 and sinking of the femoral stem in 23. Conclusion Total hip replacement is a very important and effective treatment of choice for patients with AS. The modified combination of anterolateral and lateral approach used by the authors facilitated release of contracture of the soft tissue anterior to the hip joint and correction of flexsion deformity of the hip. In case of multiple joint involvement, the surgery should be performed on the most severely involved one and the next surgery, if required, should be scheduled later within 3 to 6 months.
